We are currently accepting new patient registrations. Please visit the Clinic to pick up your registration paperwork or click the link to print and complete. Registration Form and Consent
Once you have completed your registration paperwork please bring it to the clinic for processing. You will be asked to leave your paperwork at the front reception desk and the Nurse Practitioner Lead and Administrative Lead will review and process the paperwork. You will then receive a call to advise you if you are accepted as a patient and you will be provided an appointment for your initial intake with your assigned Nurse Practitioner.
Please note you are not considered a registered patient until you attend and complete your intake appointment. We are not able to refill your medication including narcotics or controlled substances or address any of your medical concerns until after your initial intake appointment.
Please note the clinic does have a no show/late cancellation policy in effect. We require 24 hours notice for appointment cancellation or you could be charged a fee. If you are unable to attend your appointment please leave a message on our clinic cancellation answering machine if you are calling outside of regular clinic hours.
