Primary Care
Our Nurse Practitioner Clinic provides comprehensive, accessible, person centered and coordinated primary care services to registered people of all ages.
Immunization
We provide childhood, adolescent and adult immunizations to registered patients.
Lifestyle Management
For registered patients, appointments can be made with the Registered Nurse to discuss your concerns regarding weight, high blood pressure, osteoporosis, and more.
Diabetes
Diabetes education is available. This program is open to our patients with diabetes, are at risk for diabetes or to a caregiver for a person with diabetes.
Mental Health
We know anxiety, depression and stress can affect our health and wellbeing. Registered patients are welcome to call the NP clinic to make an appointment with our Registered Social Worker.
Pharmacist
Our Pharmacist is a member of your Health Care Team and works closely with your Nurse Practitioner, to ensure the most appropriate use of medication. As a register patient of the clinic, this service would be available to you.
Other Services
IUDs, Skin Concerns & Small Procedures, Prenatal Visits, and Phlebotomy (blood draws) services are available for our registered patients.
